How ICBC Treatment Funding Works: A Plain Guide for BC Patients

Most people injured in a crash in British Columbia have never dealt with ICBC before, and the paperwork arrives at exactly the moment they have least capacity for it. This is a plain explanation of how treatment funding works and what you actually have to do.

You do not need a referral to start

Under BC’s Enhanced Care model, you can begin treatment for a motor vehicle injury without a doctor’s referral. A set number of treatments across several disciplines — physiotherapy, massage therapy, chiropractic care, acupuncture, counselling and kinesiology — is pre-approved when care begins soon after the crash.

The practical implication: the barrier to starting is lower than most people assume, and waiting for a referral appointment can cost you weeks of the recovery window.

You do need a claim number

Report the crash to ICBC and you will be issued a claim number. Bring it to your first appointment. Without it, a clinic cannot bill ICBC directly and you would be paying up front and claiming back later.

What direct billing means for you

When a clinic direct bills ICBC, the treatment is invoiced to your claim rather than to you. For approved treatment within the pre-approved amount, that generally means no out-of-pocket cost at the time of your appointment.

Two things to be aware of. Some disciplines carry a small user fee on a first visit depending on the treatment type, and clinics vary in how they handle it — ask when you book. And missed appointments are not covered by ICBC, so cancellation policies apply to you personally rather than to the claim.

What happens after the pre-approved treatments

If you need care beyond the initial pre-approved amount, your practitioner submits a report to ICBC supporting continued treatment. This is routine. It is also a reason to attend consistently and to be candid at your appointments about what is and is not improving — the report reflects the clinical record.

Where active rehabilitation fits

Recovery from a motor vehicle injury usually has a passive phase and an active one. Early on, hands-on treatment does more of the work. Later, rebuilding strength and tolerance matters more, and that is where kinesiology and active rehabilitation come in. Both are covered disciplines, and planning for the transition tends to produce a better outcome than stopping when the pain eases.
